Required Documentation
The Enrollment Application and the Consent to Request/Release Records (see buttons to the right) are the only forms required to place a child on the waiting list. A child must be 5 years old by September 1st to enroll for Kindergarten. No exceptions.
Once you are notified that a spot is available, the following documents and forms are required to enroll a child:
1. Birth Certificate: Hospital Certificate or Passport
2. Social Security #: Bring original and we will make a copy
3. Report Card: Last Report Card
4. Proof of Address: Utility bill or lease agreement
5. Driver's License: Of enrolling parent/guardian
6. Custody/ Guardianship Papers: If applicable

***ALL ENROLLMENT DOCUMENTS MUST BE COMPLETED AND TURNED IN TOGETHER.
